Focus Security Requests for Local Agencies

For new employees, print the New Employee Training Checklist (policy 300.11) to use as a standardized checklist with all new employees to assure core tasks and responsibilities are completed. WIC Coordinators must complete a New User Request Form before a two-factor authentication code and access to Foucs can be provided. This form needs sent to the WIC help desk email. Focus access is only allowed for employees that are paid with WIC funds due to participant confidentiality.

Upon an employees exit from employment under the WIC program an Inactivate User Request Form needs to be completed and sent to the WIC help desk email on their last day of employment.

The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
