The document below explains the required steps to complete the reactivation process. While the process has several steps many of the requirements can be done simultaneously.
To apply for reactivation of an inactive license in Iowa complete the following reactivation form.
Reactivation of License to Practice Respiratory Care
If the license has been inactive for two or more years, the licensee shall be required to complete a mandatory criminal background check. A licensee must submit two completed fingerprint cards to the board office. Most law enforcement agencies have fingerprint cards available upon request or contact the board office at 515/281-0254 and the reactivation packet will be sent to you.
If the license has been inactive for less than two years complete the reactivation online or complete the form above.
